This person must have excellent telephone skills, medical and medication knowledge, and have experience on an Electronic Health Record.
Overview:
A service coordinator at NWMH works in collaboration with primary care providers and staff to ensure patients obtain valuable services and supplies that have been ordered.
Getting the needed supplies and care that have been started in nursing homes, hospitals or in the office is a key step in primary care that often falls through the cracks.
We are looking for someone to develop an expertise in DME and infusion ordering, as well as coordinating LTC, hospice and home health orders.
This is the first time we have looked for a person to fill this role, so the opportunities are endless to make this job your own.
SKILLS AND ABILITIES Demonstrates the ability to use a personal computer, office equipment and various software programs applicable to the position Familiarity with industry best practices and standards Familiarity with medical complexities of patients and patient charts Ability to explain complex problems & concepts to patients and providers Effective and professional communication skills, both verbal and written Exhibits the ability to maintain confidentiality, think and act independently with minimal supervision Ability to use initiative and judgment skills in carrying out responsibilities Good interpersonal skills and communication with all levels of staff and management Excellent team building and management skills Organizational skills that promote the ability to work well without direct supervision, manage time effectively and appropriately prioritize multiple assignments to get patients access to care Effective collaboration and communication with a variety of individuals, both internal and external, including multidisciplinary teams Ensures orders and documentation support adherence to all federal, state and other regulatory requirements.
Maintains regular, consistent and punctual attendance at the assigned job location.
Estimated Salary: $20 to $28 per hour based on qualifications.
